Skilled Nursing Care Services
SIR Management provides consultative services to the following skilled nursing facilities: Applewood Rehabilitation Center, Columbus Park Nursing and Rehabilitation Center, Elmwood Care, Fairview Nursing Plaza, Maplewood Care, Neighbors Rehabilitation Center, Regency Rehabilitation Center and Rock Island Nursing and Rehabilitation Center.
Rehabilitation
Individualized therapy and rehabilitation program focuses on enabling patients to return home with maximum level of functional independence. We incorporate state of art technology and professional therapists to manage and oversee your optimal level of functioning and capacity. Physical, occupational and speech therapies are offered 7 days a week, depending on resident’s physician orders.
Advanced Acute Care
Skilled medical professionals provide services such as IV therapy and individualized treatment protocols for patients who are chronically ill or have multiple medical problems or disorders. Many residents may have an existing chronic condition, such as diabetes or heart disease, that prolongs the recovery period from an illness or injury. These patients often require several therapies and services.
Physical Therapy, Occupational Therapy, Speech Therapy
Much like a hospital, residents of SIR Management consulted facilities enjoy the benefit of a full rehabilitation department. Physical, occupational and speech therapies are available by registered and licensed therapists. There is a variety of rehabilitation equipment used in our therapy department to help and support the resident’s reconditioning. Our goal is to foster independence and strengthen our residents through our services.
Alzheimer's/Dementia Care Services
SIR Management consulted facilities offer Alzheimer’s/Dementia Care is a structured, activities-based program dedicated to meeting the emotional and physical needs of people with Alzheimer’s and related memory impairments. Unlike other facilities, we offer a secured, safe and comfortable unit for our residents to feel at home, thus providing peace of mind to their family members.
Ventilator Unit
At our select facilities, we offer a unique program for pulmonary residents that focuses on quality of life and lessening dependence on ventilators and other respiratory aids. Led by pulmonologists, our respiratory staff employ state-of-the-art equipment, pharmacology and treatment protocols to provide respiratory services that meet hospital standards.
Vent/Dialysis
Ventilator/Dialysis program provides long-term care for ventilator dependent residents that actively need acute dialysis services simultaneously.
On-Site Dialysis
At our select facilities, we offer on-site dialysis. Our in-house dialysis eliminates the difficulties of scheduling transportation and time management conflicts between family members, not to mention wait time. We offer hassle free services directly in the facilities. It also alleviates the health risks associated with exposure to winter weather. Our in-house dialysis features the latest in medical technology, and a specialized dialysis nursing staff to continually monitor all resident’s progress.
Hospice Care
Compassionate, Caring, supportive care for residents with a terminal condition incorporating pain management and pastoral counseling that focuses on the providing quality of life at the end.
Bariatric Care
We provide residents with a unique, interdisciplinary team approach care with dedication to behavioral and life support of long term weight management success.
Respite Care
Short term respite stays enable caregivers to take a vacation, or to recuperate from an illness or injury without the worries of caring for their loved one. We provide peace of mind and flexibility.
Wound Care
Skin breakdown can occur quickly with the aging population due to their tissue fragility. Our comprehensive wound care program begins with preventative measures to avoid skin abrasions, pressure areas and shearing. If a skin condition should occur, our highly trained nursing, dietary and physical therapy staff work together with the resident’s physician to ensure optimal healing measures and treatment are implemented.
Adult Day Care
Flexible scheduling can provide socialization, meals, medication, activities and supervision for adults, either a few hours at a time or on a daily basis.
