Psychiatric Care Services

SIR Management consults to Albany Care, Bryn Mawr Care, Decatur Manor Healthcare, Greenwood Care, and Wilson Care.  These 5 homes are Intermediate Care Facilities for the Mentally Ill (ICF-MI).  These homes of varying size are located in North suburban Evanston, on the North side of Chicago and in central Illinois.  We provide psychiatric rehabilitation for chronically mentally ill adults in a residential setting, providing a stable, structured environment.

Collectively, we are one of the largest providers of residential, psychiatric services in Cook County and thePsychiatric Care Groups surrounding area.  We consider ourselves to be the “Experts in Psychiatric Rehabilitation” in our residential settings because of our combined 80+ years of experience in this field through Albany Care, Bryn Mawr Care, Decatur Manor Healthcare, Greenwood Care, and Wilson Care.  In fact there are only about 25 of this type of home in the entire state of Illinois, and we are proud to work with 5 of them.  Our psychiatric rehabilitation programming has a proven success record.  In addition to our comprehensive programming, our homes also provide medication management, vocational training, MISA programming, a wide variety of recreational activities and out-trips, delicious meal service and discharge planning.  We have much to offer to our residents.

All of the residents that live in ICF-MI homes have a primary diagnosis of a Mental Illness.  The residents of these homes have all been screened by a designated Department of Mental Health agency (PAS screen) and have been approved to live in this setting.  The residents have major deficits in living skills in a number of areas that prevent them from living independently and thus require the supportive services and programs of an ICF-MI.  These facilities also must provide by Federal Law, a continuous Specialized Services program for each individual.  By law, residents living in these types of homes have all of the rights and responsibilities of any other member of the community.  We take pride in the quality of care provided by our 5 ICF-MI homes.  Our psychiatric rehabilitation programs offer residents the opportunity to manage their illnesses in an environment less restrictive and less expensive than the state hospital.  Independent living in a more normalized environment contributes to an improved quality of life for our residents.

Listed below are a few of our Programs and Services provided at our ICF-MI facilities and their benefits to the resident:

Medical Services are available to the residents because medical doctors, including psychiatrists, visit the home on a regular basis.  Other medical services are also scheduled either at the facility or coordinated for the resident, along with nursing care.

Medication Management and monitoring are provided through the nursing department that is staffed 24 hours/day, 7 days/week.  The residents are assured that their medication is available, timely, and distributed correctly as prescribed by the physicians.  Individuals with a mental illness often miss medication when living in community settings.  They often do not get their medications refilled and convince themselves that they no longer need the medicine.

Meals and nutritional support are provided for specific Disease states by giving special diets to meet the individual needs of each resident.  People living in the community with a mental illness often eat poorly and infrequently.  These poor diets can affect their mental and physical conditions.

Psychiatric Rehabilitation Services are provided by bachelors and masters-level prepared professional staff, who assist the residents in meeting their stated life goals.  An individual care plan is developed collaboratively with objectives and interventions to address identified needs.  The staff meets with residents in both group and individual settings.  Problems can be identified and interventions implemented quickly to avoid relapse.  The case management staff set up a daily schedule for each resident.  This structure is an important component of our programming, which addresses: Self Maintenance, Social Skills, Community Living, Occupational Preparedness, Symptom Management, and Substance Abuse issues.

Recreational and Leisure Activities are provided to the residents by offering them the opportunity to engage in a large variety of therapeutic activities to meet their interests both inside and outside of the facility.  Residents are offered a full range of options for the mind, body, and spirit to keep active and engaged.

Rehabilitation, Discharge Planning, and Community Reintegration is provided by the case management staff who assist the residents in obtaining the skills they need to live as independently as possible.  The individual care plans identify goals and objectives for that resident to work towards.  Each resident is assessed for his/her ability to function safely in the community, with the eventual goal of being discharged to a less restrictive environment.  For those residents who are ready to live in a less restricted setting, staff provides resource materials and referrals for help outside the facility to work towards a realistic discharge plan.

We believe that without proper services and programs for those afflicted with a Mental Illness, the possibility exists that these individuals will be returned to the State Mental Health Institutions and Hospitals for care at a much higher cost than is possible in an ICF-MI setting.
