Fairview Nursing Plaza welcomes you to our home. Fairview Plaza is located in Rockford Illinois, with easy accessibility from Chicago and Southern Wisconsin. We are a 213 bed facility that has just completed our remodeling project which included new paint, wallpaper and much more.
Fairview Nursing Plaza is licensed by the State of Illinois and is approved and certified for Medicare, Medicaid, Medicaid pending, Veteran Administration and many insurance plans.
Our facility is a unique facility that specializes in care - not illnesses. Most long term care facilities specialize in either skilled nursing services or intermediate care services for the Mentally Ill. Fairview Plaza specializes in both, providing quality care to individuals, not illnesses. Our focus has been to serve our community and meet their care needs, whether they are short term rehabilitation needs or long term care, we are prepared to meet those needs.
Fairview Nursing Plaza has two floors of resident caring. The first floor is designed to meet the needs of residents with medical conditions, including our hospital step-down Medicare wing. The second floor is primarily designed to meet the needs of residents with Alzheimer's, dementias, and other psychiatric diagnoses.
